- throw_on_discarded() and throw_integer_out_of_range() take an
  input_format_t and call input_format_name() themselves
- has_no_children(), set_parents() and max_size() check array, then
  object, instead of calling is_primitive() first
- parse_impl() chains like accept_impl(); sax_parse_impl() uses if/else
- std::array for the control character messages; negation<> in
  write_object_key(); clarify the is_primitive() comment
